Our Application Process
Once you’ve reviewed and determine that you meet the criteria, please click here to submit your information to our Intake Coordinator.
After receiving your information, our Intake Coordinator will reach out to you within 48 business hours to schedule a phone screening with you. Based on the information provided in the screening, you will be notified of any additional steps that need to be taken and/or be provided with community resources to meet your need. Please understand that all steps in the application process are provisional and are not a guarantee of acceptance into our program.
Please note that being honest in your screening interview will be helpful in successfully entering our program.

Participant Criteria
To be approved for and admitted to the CLT Passport Program, you must review and meet our qualifying criteria:
You are facing homelessness, poverty, and unemployment due to a life-altering event
You are highly motivated and willing to go above and beyond for long-term change
Single adults age 18+, married couples with child(ren), or single mom/dad with child(ren)
No PENDING criminal charges; No Misdemeanors in the last 3 years. No Felonies in the last 6 years
Actively seeking a community, gain full-time employment and permanent housing
Free and clean of impairing substances and can always pass a drug test
Willing and able to be employed in a permanent full-time job (at least 30 hours per week)
Participate in assessments, programs, and counseling
Provide the Harvest Center with a copy of your Comprehensive Clinical Assessment
Willing and able to attend any classes prescribed on your Person/Family-Centered Plan
Positive attitude and willingness to give back to the community
Understand there is limited living space for your items and demonstrate flexibility and adaptability to change
