CLT Passport Program
Holistic Care and Support
The Harvest Center CLT Passport Program is a faith-based and holistic approach to supporting individuals and families facing situational homelessness.
Orchestrated by our Transformation Team, our approach is based on the 8 Dimensions of Wellness and supported by specially selected community partners. The goal is to walk with each participant through their transformational journey.
The process starts with development of a comprehensive assessment for each participant, followed by the creation of a Person-Centered Plan (PCP) or Family-Centered Plan (FCP).
Meeting weekly with our transformation specialists, our men, women, and children start to heal and become part of a community. They complete degrees, learn new skills, and prepare for jobs that will allow them to live independently.
Many of our participants are striving to overcome cycles of trauma, abuse, and poverty.

How It Works
Participants live on our campus for 12–18 months in our residential program that is holistic, Christ-Centered, and life-changing. At the end of their program, each individual participates in a Phase Up Ceremony, where stories of transformation are shared and we celebrate together.
Individuals not only gain the resources and skills needed to support themselves, but they have an abundance and the natural response is to give back to others in return.

The Harvest Center is more than a resource center, or a residential facility; it’s a true community focused on healing and future opportunities for all. We focus on healing from trauma, while addressing the eight core dimensions of wellness.
In partnership with donors, volunteers, churches, corporate partners, and civic partners; the Harvest Center bridges hope and home, helping participants rise above homelessness, trauma, and poverty.
