Job description: The Transformation Specialist is the cornerstone of The Harvest Center of Charlotte’s Transformation Passport Program. This role walks alongside individuals and families as they pursue stability, healing, and long-term transformation. Specialists create and manage Person Centered Plans (PCP) or Family Centered Plans (FCP), grounded in the 8 Dimensions of Wellness, while modeling firm, loving accountability and a commitment to each participant’s full potential.

Transformation Specialists embody THCC’s core values, maintain professional boundaries, and ensure every participant receives consistent, compassionate, and structured support.

EDUCATION/EXPERIENCE/QUALIFICATIONS

  1. Education
    • Bachelor’s degree in Social Work, Psychology, Human Services, or related field
  1. Experience
    • Minimum one year of case management and documentation experience
    • Experience working with individuals experiencing homelessness
    • Strong communication, coaching, and leadership skills
    • Proficiency in Microsoft Office
